All-Nighter Terms
Parents/Gaurdians must leave phone number that WILL be answered the night of the event. In the event of disciplinary problems, children must be picked-up within 30 min of a phone call. Pick-up is 6:30am-7:00am. There will be a late fee of $5 per child each 15 min. pick-up is late.

Consent and Liability Waiver
Consent and Liability Waiver: Parent or Guardian must sign. I, (parent/guardian), desire for my (son/daughter) to participate in one of Charlotte Sports Center, LLC's youth activities. I understand that sports involve physical contact between participants and I assume the risks associated with (his/her) participation. My child has no known medical conditions which prohibit participation in this summer sports camp. I agree to hold harmless and release Charlotte Sports Center, LLC and their agents and sponsors from legal liability resulting from any injuries sustained during participation in these activities. I understand Charlotte Sports Center, LLC cannot be responsible for loss or damage to my child's personal possessions. You may use my child's photograph or video in future promotions. I understand that no refunds will be granted for absences, withdrawals or illness.
